DataM Intelligence’s report analyzes therapy, stage, end-user and regional segments, including pipeline developments, company positioning and country-level trends. Competition will increasingly depend on HER2 testing, treatment sequencing and the expansion of newly approved or clinically advancing regimens across markets.

Leander, Texas, United States, October 21, 2026 /MarketersMEDIA/ -- The global HER2-positive gastric cancer market is valued at US$1.31 billion in 2025 and is forecast to reach US$2.05 billion by 2035, at a 4.6% CAGR, according to DataM Intelligence. The treatment landscape is moving beyond a trastuzumab-led approach: antibody-drug conjugates (ADCs), immunotherapy combinations and bispecific HER2 antibodies are creating new treatment choices across disease stages and lines of therapy.

The FDA’s August 2026 approval of zanidatamab-based regimens puts HER2 test results at the center of treatment selection and creates a new competitive challenge for established trastuzumab combinations. In Japan, where gastric cancer burden is high and trastuzumab deruxtecan has moved into second-line treatment, the next question is how quickly emerging first-line regimens can gain local approval and access.

United States

On August 25, the FDA approved zanidatamab with chemotherapy and tislelizumab for first-line treatment of eligible HER2-positive, unresectable locally advanced or metastatic gastric, gastroesophageal junction (GEJ) or esophageal adenocarcinoma. Eligibility includes IHC 3+ or IHC 2+/ISH-positive disease. Zanidatamab with chemotherapy alone is approved for IHC 3+ tumors. The FDA also approved companion diagnostic tests to identify eligible patients. This makes reliable HER2 testing essential to access and gives pathology labs a more direct role in treatment selection.

Japan

More than 125,000 gastric cancer cases and over 43,000 deaths were recorded in Japan in 2022, according to Daiichi Sankyo’s release on the country’s treatment update. In March 2026, Japan expanded trastuzumab deruxtecan (Enhertu) to second-line treatment for eligible HER2-positive advanced or recurrent gastric cancer, advancing it from its previous third-line position. Japan also participated in the global HERIZON-GEA-01 trial; the Japanese trial registry recorded Ehime Prefecture as a study location and listed the study as no longer recruiting in its October 15 update. U.S. approval does not establish Japanese approval, so local regulatory status remains a separate commercial consideration.

Recent Developments

  • Phase III results strengthened the case for a new first-line HER2 approach. Results from HERIZON-GEA-01, published on May 27, 2026, showed median progression-free survival of 12.4 months for both zanidatamab-based treatment arms, compared with 8.1 months for trastuzumab plus chemotherapy. The arm combining zanidatamab, tislelizumab and chemotherapy reported median overall survival of 26.4 months, versus 19.2 months in the control arm.
  • PD-L1-negative patients remained part of the treatment discussion. Data presented at the 2026 American Society of Clinical Oncology meeting reported benefit with zanidatamab, tislelizumab and chemotherapy across PD-L1 subgroups. This may affect how companies plan testing and communicate the role of PD-L1 alongside HER2 in first-line treatment decisions.
  • Diarrhea management is an operational consideration. In HERIZON-GEA-01, diarrhea was the most common grade 3 or higher treatment-related adverse event in the zanidatamab-containing arms. The study used anti-diarrheal prophylaxis during the first cycle; most events occurred early. Treatment implementation will therefore depend in part on clear monitoring and early intervention protocols.
  • Japan registered a study for tumors expressing both HER2 and CLDN18.2. The ZAP-HETERO phase II study, registered in June 2026, is evaluating zolbetuximab, trastuzumab and CapeOX, with pembrolizumab use guided by PD-L1 expression. The trial reflects a research question specific to biomarker overlap; it is investigational and does not establish an approved treatment.
  • Japan approved perioperative durvalumab for gastric cancer. On June 19, 2026, Japan approved durvalumab for pre- and post-operative treatment of gastric cancer. The approval is not specific to HER2-positive disease, but it signals activity in earlier-stage treatment and may influence how companies assess future perioperative opportunities.
  • The FDA approved two first-line zanidatamab regimens on August 25. Zanidatamab with tislelizumab and chemotherapy is approved for HER2 IHC 3+ or IHC 2+/ISH-positive disease. Zanidatamab with chemotherapy alone is approved for IHC 3+ disease. The distinction makes the testing threshold central to treatment eligibility.
  • The FDA also approved companion diagnostic devices. The PATHWAY anti-HER-2/neu (4B5) antibody and VENTANA HER2 Dual ISH DNA Probe Cocktail were approved to identify patients eligible for the zanidatamab regimens. The decision links treatment access directly to validated testing workflows.

Market Segmentation:

Targeted therapy represented 38.6% of 2025 market revenue, equal to about US$506 million on the report’s US$1.31 billion market base. This segment includes HER2-directed treatment such as trastuzumab and ADCs. First-line bispecific antibody and immunotherapy combinations are now adding competition within this established targeted-treatment pool.

Stage IV disease accounted for 41.2% of 2025 revenue, or about US$540 million on the same base. The segment’s share reflects the use of systemic therapies in advanced disease, including multiple treatment lines. New first-line approvals and second-line ADC access in Japan may reshape how revenue is distributed across regimens and treatment stages.

Company Profiles

Jazz Pharmaceuticals commercializes zanidatamab in the United States and other territories covered by its license. The FDA’s first-line approval gives Jazz a broader role in HER2-positive gastroesophageal adenocarcinoma beyond the drug’s existing biliary-tract-cancer indication. Execution will depend on uptake of the new combination regimen and on access to HER2 testing.

BeOne Medicines supplies tislelizumab (Tevimbra), the PD-1 inhibitor included in the approved triplet regimen. Its collaboration with Jazz on HERIZON-GEA-01 and the trial’s published results position the company as both a treatment partner and a contributor to first-line clinical evidence.

Zymeworks engineered and first developed zanidatamab, then licensed development and commercialization rights to Jazz and BeOne by territory. Its 2025 Form 10-K describes potential tiered royalties and milestone payments tied to the partners’ sales and regulatory progress, giving Zymeworks an ongoing economic interest in the drug’s commercial expansion.

Daiichi Sankyo and AstraZeneca jointly develop and commercialize trastuzumab deruxtecan (Enhertu). In Japan, Daiichi Sankyo commercializes the ADC and expanded its gastric-cancer use to second-line treatment in March 2026. The product remains a central comparator as new first-line combinations enter the market.

Yuri Yoshida wins in South Korea for her first LPGA Tour victory

HAENAM, South Korea — Yuri Yoshida won the LPGA South Korea tournament holding off Haeran Ryu and Hye-Jin Choi in a close finish at Pine Beach Golf Links on Sunday. It was Yoshida's first LPGA Tour victory and came in dramatic finale as both overnight leader Haeran Ryu and Yoshida bogeyed the final hole leaving the Japanese player ahead by one stoke and a four-round total of 17-under 271. Yoshida was four shots off the pace going into Sunday’s final round but lit up the front nine with an eagle at the par-5 second, followed by three consecutive birdies to be 5-under 31 at the turn. Bogey’s at the 15th and 18th was offset by a crucial birdie at the 17th which gave the 26-year-old the buffer she needed. Ryu led the tournament by four strokes going into the final round, but couldn’t find the same rhythm on Sunday as she shot a 73 to finish at 16-under 272. The South Korean player made 15 consecutive pars before dropping a crucial shot at the par-4 16th which handed back Yoshida’s one-shot advantage. Both player's made birdies at the 17th, but a costly miss for Ryu at the 18th allowed Yoshida to clinch it, despite the Japanese golfer also making bogey. Hye-Jin Choi, who was in a tie for second coming in the final round, stayed in contention throughout Sunday and mixed four birdies with a lone bogey to finish at 16-under 272, and a shot behind Yoshida in a tie for second with Ryu. Choi is still looking for her first victory in 125 career LPGA Tour starts as a member. She has held or shared a 54-hole lead five times in her LPGA career without converting it into a win. Last week in Shanghai, Choi was runner-up by one stroke to China’s Ruoning Yin after the South Korean golfer led after each of the first three rounds. Defending champion Sei Young Kim, who ended a five-year victory drought in 2025 with a win last year in South Korea, shot 70 Sunday and finished at 2-under 286 and a tie for 55th. In its late-season Asian swing, the LPGA Tour moves to Kuala Lumpur, Malaysia next week for the Maybank Championship, the third of four events.

Booker scores 43 points in Suns' 131-122 win over Warriors

PHOENIX — Devin Booker scored 43 points, Miles Bridges added 16 and the Phoenix Suns won their home opener for the fifth straight season by beating the short-handed Golden State Warriors 131-122 on Saturday night. The Warriors — who were playing for the third time in four days — fell to 0-3 for the first time in 19 years while resting several key players, including Stephen Curry (right knee), Brandin Podziemski (low back), Draymond Green (low back) and Al Horford (low back). Booker scored 17 points in the third quarter and Jordan Goodwin hit a 3-pointer just before the buzzer to give the Suns a 102-92 lead entering the fourth. The Warriors hung around for a few more minutes, but the Suns eventually pulled away and Luke Kennard hit a 3 to extend the lead to 117-102 with six minutes left. Kennard finished with 11 points, hitting 3 of 5 from 3-point range, which is exactly what the team expected when they signed him in the offseason. Dillon Brooks had 15 points and five assists. The Suns were coming off an embarrassing 124-83 road loss in the season opener to the Trail Blazers. They struggled in the opening quarter of Saturday's game, too, making just 1 of 12 shots from 3-point range. But the shots finally started to fall and it was Booker who led the way. The five-time All-Star made 17 of 25 from the field and added seven assists and seven rebounds. Golden State was led by rookie Yaxel Lendeborg, who scored a season-high 26 points and made six 3-pointers. De'Anthony Melton added 25 points on 8 of 13 shooting. Booker scored 19 points in the first half, leading the Suns to a 57-54 halftime lead. Lendeborg had 13 for the Warriors.

Struggling Altas take on Chiefs, seek to end slump

With four games remaining in the elimination round and drifting on a six-game losing streak, University of Perpetual Help seeks to move higher in the Group B standings and make the quarterfinal round of the NCAA Season 102 men's basketball tournament. The Altas, however, need to win games, starting with its 5 p.m. duel with the Arellano University Chiefs on Monday at the FilOil EcoOil Centre in San Juan City. Perpetual Help currently sits at fourth place in the five-team Group B with a 2-7 win-loss record, ahead of winless San Sebastian (0-10). The Chiefs are at the number two spot with a 5-4 card. The other two teams in the group are pace-setter Letran (7-2) and third ranked Emilio Aguinaldo College (2-6). The Altas aim to finish the elims at least in third place to get an outright quarterfinals berth. The fourth and fifth placed teams will clash in a one-game playoff for a seat in the quarterfinal round. But more than the thought of advancing to the next round, the Altas need to get their acts together and play as a solid unit. Coach Olsen Racena needs Mark Gojo Cruz, LA Casillo, Ray Allen Maglupay, JM Tulabot, Jim Corpuz, Gelo Gelsano, Ariea Borja and Shawn Orgo to play well on both sides of the court to avenge Perpetual Help’s 88-65 first round loss to Arellano University on Monday. The Junior Altas, on the other hand, are in a better position in the juniors division. The Junior Altas enters their game against the Arellano University Braves with a 7-2 record in Group B. However, the team coached by Joph Cleopas is coming from an 82-86 setback against Group A leader Jose Rizal University. Still, Perpetual Help is heavily favored to win against Arellano which the Junior Altas defeated in the first round, 104-97. Expected to lead the charge for the Junior Altas are Louie Rosales, Sizco Roquid, Brian Orca Jr., Lord Dulanas, Sam Villaruel, Arkin Cristobal, Kakai Zamoras and Kurt Patalinghug.
